No Refunds on Registration Fees. If any debt is left owed to Clanton Tiger Orange for the previous year, this amount and
any additional cost, for the current football season must be paid in full before the child is allowed to participate in the
program.

I, fully understand that the Clanton Tiger Orange Foolball Organization operates in the Chiton Youth Football League and that I and the player/cheerleader candidate are subjected to the Rules and Regulations of the Chilton Youth Football Organization.

I, parent or guardian of the above named is a candidate for a position on the Clanton Youth Football Program, hereby give approval to his or her participation in any and all League Activities during the current year.

I, assume all risks and hazards incidental to such participation including transportation to and from the activities, and do hereby waive, release, absolve, indemnify and agree, to myself individually and as the parent or guardian to waive any liability on the part of the City of Clanton, Chilton County High School, Clanton Middle School, and Clanton Tiger Orange Youth Football League Program

I, also grant permission to managing personnel or their league representatives to authorize and obtain medical care from any licensed physician, hospital, or medical clinic should the child become hurt or injured while participating in league activities, should neither parent or a guardian be available to grant authorization for emergency treatment.

I, agree to return upon request the uniform, and other equipment issued to the child. I will assume the entire cost of replacing equipment if not returned upon request. I will furnish a certified birth certificate for the above named Football Player upon request by league officials.

I, fully understand that the Clanton Youth Football League is not offering insurance. Insurance on the above named candidate is my full responsibility.

Punishment for any infraction of these rules will be dealt with by dismissal of parents and their child from the Clanton Tiger Orange Youth Football Program, or not allowed to participate in the next game. This applies to cheerleaders as well as football players.

I have read and I understand and agree with the rules above on this application. By my signature, I will abide by each one of them. I understand that all decisions made by the Board of Directors are final. By not abiding by conditions of the organization, I understand that I could be dismissed from the league and be asked to leave.
